Gravity Vault Waiver Form

Every Climber in our facility must have a completed waiver form prior to climbing.  If you are under 18 years of age one must be completed by a parent or guardian prior to climbing. 

Birthday Party and Intro Waiver (pdf) (two page pdf) -This form is appropriate for those working with our staff: Birthday Parties, Introduction to Rock Climbing, Walk-Ins, and Staff Belay climbers.  This form is also available under the Kids' Parties section and is for one day and must be completed each time you climb with us.

If you climb frequently with our staff, the form below needs only to be signed and submitted once with an electronic signature upon each subsequent visit.

The Gravity Vault Waiver (four page pdf) - This form is appropriate for ALL climbers.
