Gravity Vault Waiver Form

Every Climber in our facility must have a completed waiver form prior to climbing.  If you are under 18 years of age one must be completed by a parent or guardian prior to climbing. 

There are two versions of this form.  The below form is appropritate for ALL climbers:

There is also a shorter version appropriate for birthday parties, walk-in climbers and those working in staff belays.  That form is available under the Kids' Parties section, all other climbers should fill out the above waiver.
